Who to Call
Dr. Hachadorian cares for patients through his independent practice and through Kaiser Permanente. The simplest way to reach the right team: if Kaiser was involved in your care or surgery, call Kaiser — otherwise call the independent practice.
Seen through the independent practice
If Dr. Hachadorian cared for you through his independent practice — that is, not through Kaiser — call the office directly.
Seen at Kaiser
If you were seen at any Kaiser location, or your surgery was done through Kaiser, call Kaiser. Your call-back line can vary by procedure, so the surest contact is the one given to you directly.
Call the phone number on your discharge paperwork or after-visit summary.
For non-urgent matters, message your care team at kp.org or in the Kaiser Permanente app.
Not sure which one applies to you?
The simplest test is whether Kaiser was involved:
- Seen at a Kaiser facility, or surgery arranged by Kaiser? You’re a Kaiser patient — call Kaiser.
- Carry a Kaiser Permanente member card? Your care was most likely arranged through Kaiser.
- None of the above? Your care was through the independent practice — call the office.
